PRE & POST OP INSTRUCTIONS

At Stephens Plastic Surgery & Skincare Aesthetics of Baton Rouge, your health and wellbeing are extremely important to us — and, of course, your results. All of this can be greatly helped when you closely follow pre- and post-care instructions before and after a surgical procedure.

Dr. J. Anthony Stephens and his team are well-known for taking exceptional care of patients, and when you choose to work with us, you can be absolutely sure that you’re in the best of hands. We encourage you to carefully adhere to the pre- and post-op guidelines and tips we have provided here. Doing so will ensure a smoother, safer recovery. CANCELLATION POLICY

• In order to be respectful of our staff and other patients, we are requesting a 24-hour notice for all appointment cancellations.
• Repeat 24-hour cancellations and "no shows" may result in pre-payment requirements for rebooking.
• If an appointment results in consecutive cancellations/reschedules, a minimum charge of $25 will be required and collected as a pre-payment for the next appointment, starting after the 2nd cancellation/reschedule.
• If an appointment is missed without any communication (deemed a “no show”), a minimum charge of $25 will be required and collected as a pre-payment prior to re-booking any future appointments; to be charged at the provider’s discretion.
• Fees collected due to the above stated policy may change at any time and are circumstantial (fees may increase given continued cancellations or if longer appointment times are booked).
• If a fee is collected, this will be used at the next scheduled office visit and applied towards your service. However, if this rescheduled appointment again results in a cancellation/reschedule, you will lose your minimum $25 deposit and another charge of $25 will be required and collected as a pre-payment for your next appointment.

SURGERY PROPOSAL

• THE FEES INDICATED ARE FOR THE PROCEDURES LISTED BELOW.
• THESE FEES WILL BE HONORED FOR 6 MONTHS FROM TODAY'S DATE.
• ALL FEES MUST BE PAID 21 DAYS PRIOR TO THE SCHEDULED DATE OF THE PROCEDURE.
• FEES MAY BE PAID BY CASH, CHECK, VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS, PATIENT FI AND CARE CREDIT.
• FEES WILL NOT BE REFUNDED IF SURGERY IS CANCELED WITHIN 21 DAYS OF SURGERY.
• PROCEDURES REQUIRE A 10% NON REFUNDABLE DEPOSIT AT TIME OF SCHEDULING.
• THERE IS AN ADDITIONAL $350 FEE FOR CHANGING THE DATE OF THE PROCEDURE WITH LESS THAN 28 DAYS NOTICE.
